- New applications: Complete application, print, sign, and mail or fax; proposal form can be submitted by e-mail (preferred), mail, or fax
- Renewals: Complete application, print, sign, and mail or fax; proposal and report forms can be submitted by e-mail (preferred), mail, or fax
- Amendments: submit in writing by e-mail (preferred), mail, or fax any changes to be made
- Application form needed: Form 18-A
- Proposal form needed: Form 18-B please e-mail scpermits@azgfd.gov or call 623-236-7625 with questions.
- Report form needed: Form 18-C
- Fee: none
- Permit valid: calendar year
- Additional information needed: Detailed description of holding facilities if applicable (pursuant to R12-4-428)
